POTS Symptom Form - a two sided form that can be used to list symptoms and other important info for ER or office visits. This form also explains the basics of POTS for the doctor and explains where they can obtain more information.

POTS Medical Accommodation Card - print out this card and carry it in your wallet. If you need to sit or lay down and the people around you aren't understanding, you can show this card to people and hopefully they will allow you a comfortable place to sit or lay down.
